Dermal fillers are minimally invasive, gel-like injectables used to restore lost volume, smooth out wrinkles, and contour facial features. At AccessMedicine MedSpa our philosophy is to avoid heavy, overfilled looks and focus on "tweakments" — using smaller, strategic amounts of filler to achieve subtle, natural results that prioritize balance and skin hydration. Dermal Fillers
How Fillers Work
As we age, our bodies naturally lose subcutaneous fat, bone density, and collagen. This leads to sagging skin, hollow areas (like the temples or under-eyes), and deeper facial folds. Fillers are injected at varying depths — from deep placements on the bone to structural lifting, down to superficial micro-droplets near the surface — to physically fill these spaces or stimulate your body's own structural proteins.
The Main Types of Fillers
Not all fillers are the same. They use different chemical bases designed for specific areas of the face and body.
| Filler Type | Primary Ingredient | How It Works | Best For | Typical Longevity |
| Hyaluronic Acid (HA) (e.g., Juvéderm, Restylane) | Hyaluronic Acid (a naturally occurring sugar that binds water) | Physically plumps the area and boosts hydration. Completely reversible with an enzyme called hyaluronidase. | Lips, fine lines, under-eyes, cheeks | 6 to 18 months |
| Calcium Hydroxylapatite (CaHA) (e.g., Radiesse) | Mineral-like compound found naturally in bones | Denser gel that provides immediate lift while triggering natural collagen production. | Deep wrinkles, jawline contouring, hands | 12 to 18 months |
| Poly-L-lactic Acid (PLLA) (e.g., Sculptra) | Biocompatible, biodegradable synthetic substance | A biostimulator. It doesn't provide instant volume; instead, it prompts your body to build a new collagen matrix over several months. | Hollow temples, sunken cheeks, broad facial volume loss | Up to 2+ years |
| Polymethylmethacrylate (PMMA) (e.g., Bellafill) | Synthetic microspheres mixed with bovine collagen | A semi-permanent filler. The collagen provides instant volume, while the microscopic beads remain to give permanent structural support. | Deep nasolabial folds, pitted acne scars | 5+ years |
Popular Treatment Areas
-
Mid-Face & Cheeks: Restoring volume to the mid-face lifts the lower face, softening the smile lines (nasolabial folds).
-
Lips: Enhancing border definition, correcting asymmetry, or adding subtle volume. Modern "dynamic" HA fillers move naturally with your expressions so the lips don't feel stiff.
-
Jawline & Chin: Sculpting a more defined profile or projecting a weak chin.
-
Tear Troughs: Filling hollows under the eyes to minimize a tired appearance.
-
Skin Boosters: A newer technique where micro-droplets of thin HA are injected across the face strictly for deep hydration and a "glow" rather than adding structural shape.
Key Risks & Considerations
While fillers have an excellent safety record when performed by qualified medical professionals, they are still medical procedures.
Minor bruising, swelling, and redness at the injection site are completely normal and usually resolve within a few days. However, the most severe (though rare) risk is vascular occlusion, which happens if filler accidentally enters a blood vessel and blocks blood flow. Using a highly experienced injector who understands facial anatomy minimizes this risk and ensures they can quickly use a reversing agent if a complication arises.
